Description

Requirements
1. Develops nursing care plans for residents and maintains current documentation for prescribed medications and health care services.
2. Oversees medication pours and provides oversight to the AMAPs.
3. Provides effective guidance, counseling, training and communications (both oral and written) to residents’ direct care staff to ensure compliance with nursing care plans and the administration of medications.
4. Reviews and updates the health care status of residents and implement required changes.  Initiates and documents monthly reviews of residents’ medication administration records.
5. Establishes effective and collaborative working relationships with clinical and direct service providers both inside and outside of Arc.
6. Maintains current information related to NYS and Federal regulations and guidelines related to the medical and health care requirements for people with developmental disabilities.
7. Conducts at least weekly site visits to individual’s residences to confirm accurate and safe administration of medications.  Confirm security and safety requirements for the storage of medications.
8. Ability to drive to assigned home locations in Onondaga County and accompanies residents on health care appointments and testing as needed.
9. Owns or leases a reliable automobile to provide transportation required for this position. The automobile must be available to provide safe transportation for self, residents and/or staff.
10. Must maintain job-required certifications and/or licenses.
11. Must carry an Agency provided cell phone at all times during normal work hours and provide after hour coverage on a rotational basis.
12. Participates in and /or chair committees as assigned by the Nursing Supervisor.
13. Must be flexible to assess medical situations and/or injuries on off-hours at program sites.
14. Available to attend consumer’s 6 month and annual reviews.
15. Supports the Goals and Mission of Arc of Onondaga County.  Ensures that your communications and personal performance reflect the Agency’s commitment to teamwork, compliance with policies and procedures and respect for residents and staff.
16. Computer and office technology expertise that is required to fulfill job responsibilities and assignments.
17. Attends and participates in required Arc meetings, staff development and training programs.
18. Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.
19. Expected to conduct all agency business and complete job responsibilities in accordance with agency Code of Conduct and Corporate Compliance Plan.
20. Provides other job-related support and services as required or assigned by management.
Note – Requirements 1 -18 are Essential Job Requirements

 

Education & Experience Requirements
• Academic Degree in Nursing plus Certification as a Registered Nurse.
• Certified in CPR
• Preferred one year experience with developmental disabilities population and case management experience.
• Combination of education and experience which, in the judgment of the employer, equals above standards.
• A valid NYS driver’s license.
