What is Service Coordination?
 
Service coordinators assist individuals with developmental disabilities in obtaining the services and supports necessary for them to lead successful, independent lives in the community. Medicaid Service Coordination is a Medicaid State Plan service administered by the New York State OPWDD office. 
 
MSC’s use a “person-centered approach,” focusing on the needs and desires of the individuals they are working with and helping them to create a vision of a desirable future.  This approach seeks to build on an individual’s abilities and skills, rather than concentrate on limitations.
 
MSC’s develop, implement, and maintain Individualized Service Plans (ISP’s). ISP’s reflect the informed choices of the individual by outlining the help he/she wants and needs to achieve their goals.
 
Service Coordinators:
 
Help Individuals explore what they want and need in their daily lives and assist them in achieving it
 
Advocate for the rights of individuals with developmental disabilities on many topics including health and safety issues in their surrounding environments
 
Ensure individuals are aware of their ability to choose
 
Share their knowledge about available community resources
 
Focus on the quality of life for people with developmental disabilities and ensure that each individual receives optimal services
 
Service Coordinators meet with individuals regularly. Together, they specify the wants and needs of each individual and target specific activities and providers to help them meet their personal goals.
 
To be eligible to receive Medicaid Service Coordination Services a person must:
  • Have a documented diagnosis of a developmental disability
  • Be enrolled in Medicaid
  • Demonstrate a need for ongoing comprehensive service coordination
  • Give consent, or have a person authorized to give consent, to receive Medicaid Service Coordination
  • Not live permanently in any Medicaid funded setting that provides service coordination
  • Not be enrolled in any other comprehensive Medicaid long-term service coordination program
Family Reimbursement Program
 
Respite reimbursement is designed to assist consumers and their family members access to respite opportunities. These opportunities allow the family to have some relief from full-time care giving. Additionally, respite reimbursement provides increased opportunities for consumers' social interaction. The delivery of all FSS services are evaluated through direct contact with families as well as through the completion of customer satisfaction surveys.  Click here for a Family Reimbursement Application.
