| Summary
The Habilitation Specialist is responsible for the operation of the assigned area, including incorporating individual program plans into a daily program of activities. The Habilitation Specialist is responsible for providing direct care to consumers attending the Day Habilitation program. This position is responsible for providing hands-on assistance, supervision, instruction, and guidance. The Habilitation Specialist implements assigned goals, objectives, and services for consumers. Reports to the Program Manager.
Requirements
1. Provides hands-on assistance, instruction, and guidance on an individual, small, and large group basis in the development of skills, as outlined in program plans, to consumers in various settings as appropriate.
2. Assists consumers with personal care and hygiene needs as needed, including toileting, dining, showering, dressing, and grooming. Demonstrates appropriate techniques in all physical interactions or interventions with consumers, including dining, lifting, transferring, positioning, or restraining.
3. Implements plans, objectives, and activities, and provides services in accordance with the individual’s program plan. Consistently implements and adheres to Behavior Management Plans. Receives instruction, guidance, and direction from clinical staff on particular methods, treatments, etc.
4. Acts as a member of the treatment team, providing feedback, suggestions, etc. for program plans for consumers. Assists, as requested, in assessment and evaluation of individual’s needs and abilities.
5. Accompanies consumers on community outings, as requested. Ensures habilitative needs are addressed. Serves as a positive role model for consumers.
6. Assists in providing a safe environment for consumers, through timely reporting of facility or equipment maintenance or replacement needs, and participating in building evacuation drills. Helps maintain a sanitary environment of assigned unit, by performing cleaning responsibilities, timely reporting of needed equipment or supplies, and routine practice of infection control procedures.
7. Meets all record keeping and documentation requirements.
8. Responsible for providing safe transportation as necessary or required to consumers to and from off-site locations in agency-owned and/or own vehicle.
9. As requested, participates in program-wide Committees, and is responsible for sharing information with team.
10. Support the Goals and Mission of Arc of Onondaga County. Ensure that your communications and personal performance reflect the Agency’s commitment to teamwork, compliance with policies and procedures and respect for consumers and staff.
11. Computer and office technology expertise required to fulfill job responsibilities and assignments.
12. Attend and participate in required ARC meetings, staff development and training programs.
13. Establish and provide clear and legible documentation and compliance records required by ARC and other government agencies.
14. Provide other job-related support and services as required or assigned by management.
Education & Experience Requirements
· High School Diploma/GED.
· One-year experience in human services field.
· Valid NYS driver’s license and a driving record acceptable to the Agency’s insurance carrier. |
