Summary

The Community Habilitation Specialist is to provide support services to consumers in an individualized environment tailored to the consumer’s wants and needs with a goal of independence, productivity and integration. Reports to the Day Habilitation Program Manager.

 Requirements

1.       Provides primary individualized care as outlined in the consumer’s PISP (Preliminary Individual Service Plan) and ISP (Individualized Service Plan).

2.       Assists and accompanies consumers to promote independence in the primary activities of daily living (bathing, toileting, cooking, cleaning, etc.) as described in the consumer’s ISP.

3.       Maximizes consumer’s social skills in order to enable him/her to live in their community by providing activities in community settings.

4.       Acts as a liaison with churches, clubs and other organizations to develop natural supports for the consumer in the most integrated environments possible in community settings.

5.       Works with the family, consumer and Service Coordinator to identify the family/consumer’s likes and dislikes along with establishing long term goals for consumer, family or correspondent.

6.       Maintains progress notes and reports on assigned consumers and submits to Day Habilitation Plan Coordinator on a regular basis.

7.       Attends all established or assigned meetings relative to consumer/family/correspondent or training as appropriate.

8.       Maintains confidentiality of all persons served and contributes to the improvement of Agency operations.

9.       Works flexible hours, days, evenings, weekends and holidays as necessary in order to meet the needs of individuals served.

10.   Must be able to transport self and consumers in an Agency provided vehicle and/or their own vehicle.

11.   Supports the Goals and Mission of Arc of Onondaga County. Ensures that communications and personal performance reflect the Agency’s commitment to teamwork, compliance with policies and procedures and respect for consumers and staff.

12.   Have the philosophy that their role is to teach, not parent. Assists the consumers in being as productive and independent as possible, allowing the consumers to be individuals and included in the work and social networks of their choice.

13.   Computer and office technology expertise that is required to fulfill job responsibilities and assignments.

14.   Attends and participates in required Arc meetings, staff development and training programs.

15.   Establishes and provides clear and legible documentation and compliance records required by Arc and other government agencies.

16.   Provides other job-related support and services as required or assigned by management.

Education & Experience Requirements


·         High School Diploma/GED. 

·         One-year experience in human services field.

·         Valid NYS driver’s license and a driving record acceptable to the Agency’s insurance carrier.
